Are you hosting the form on your own PC?
If you are using Linux, you need to install php and mysql. Use apt-get to get those.
If you are using Windows, an easy way to set those up is using WAMP5
http://www.wampserver.com/en/
Install that then drag the phpbb2 folder into the WWW folder (or whatever your webroot is.)
Point your web browser to 127.0.0.1 and then click on phpbb2. It should take you through the installation. You may need to create a MySQL database (I'm not too sure, not too much experience with phpBB) but you can do that with phpMyAdmin which is included with the wamp server.

Or get free webhosting that supports mysql and PHP like freehostia (freehostia.com) and then upload all the files there and go from there.

go to 127.0.0.1 and on the side where it says Projects, there should be a link above it which says phpmyadmin

Alternativley you can just left click on the tray icon for the WAMP server and select phpmyadmin
