The Window is from foobar I got the icons from Deviant art, very nice player uses very little resources.

Sweet, another foobar user.

To get album art to show up, get foo_uie_albumart from here http://cherans.bol.ucla.edu/foo_uie_albumart0175.zip
Put the .dll in your components folder.
In the preferences, under display select columns UI then select the layout tab. From there you can add on the album art by adding a splitter.
